Crèche

• To provide snacks/meals for my child. In the event that I choose to use the school’s kitchen, I will pay the costs incurred.
• I fully accept that I shall provide nappies/wipes/changing of clothing for my child, and these will be clearly labelled.
• Due to the age group of my child, I agree that the crèche staff can administer Calpol (1 spoonful) should break my child’s fever during the day. This is so that my child is protected while I come to collect. In the event that parent/carer does not wish for Calpol to be administered in this extreme situation the parent/carer takes full responsibility and must come to collect the child as soon as practically possible.
• I agree to define my child’s dietary needs to the staff, allergies or any other needs, including behaviour: biting, hitting, bumping of head, etc.
These are very normal patterns for some very young children, but the staff need to be aware.
• I agree to inform the staff of any accident/fall my child has had within 48 hours outside the crèche:
• If my child is ill, a doctor’s certificate will be provided before he/she can return to the creche.
• I will provide milk/milk formula and bottles for the duration of the day. I will guide the staff how to mix it, use it.
• I am aware of the GDPR + confidentiality policies of the whole school and agree with the policies.
• I agree to closely collaborate with my child’s key worker to ensure that my child’s Care plan is completed accurately + sign that I did so.
• I will complete the photography consent form and return to the crèche with this application form.
