ISN Application Form

Please upload any supporting documentation including the following documents necessary for processing your application:
1. An original copy of the child's birth certificate.
2. Previous school reports (if any).
3. A recent photograph of child/children.
4. If child is on any long-term medication, a doctor's certificate is necessary for confirmation.
5. A medical certificate by your paediatrician.
6. Pupils with Learning challenges need to provide a recent assessment report.

Please note that in case of an accident where medication may have to be given to the child, parents will take full responsibility should the child be allergic to the medication given, unless the school has had prior confirmation of the fact in writing.

Complete this section if you wish your child to take the bus to/from school.
The school mini bus service runs daily as follows:
• 2 Buses am
• 2 Buses 1:30pm
• 1 Bus 2:30pm
• 1 Bus 4:00pm

Complete this section if you wish your child to participate to Homework sessions or for Extended Day.
• Homework Sessions (Year 1 & above) run between 2:45-4pm. Mon-Thurs per month.
• Extended day (Nursery or Reception) runs between 1:30-6 pm Mon-Fri per month.

During your child’s time at ISN we may wish to take photographs of activities that involve your child.
The photographs may be used for displays, publications and on the school website, on social media (the School’s Facebook Page) or by local newspapers.
Photography or filming will only take place with the permission of the Senior Management Team.
Please be advised that no full names will be displayed with the picture of any student.
Names may be shown on portions of the website where pictures are not displayed.
Pictures of students may be displayed but in no case will the full name of a student be placed with a picture in a way that can identify the student, unless celebrating an achievement.
Before taking any photographs of your child, we need your permission.
You may withdraw your consent, in writing, at any time. Permission will last for the duration of your child’s time at ISN.
